This privacy policy is to provide information to you, our client, on how your personal and health information is collected and used within our clinic.  All personal data is dealt with sensitively and in the strictest confidence internally and externally.

When you register as a client of our clinic, you provide consent for David to access and use your personal information so he can provide you with the best possible healthcare.  Only authorised staff who need to see your personal information will have access to it.  If we need to use your information for anything else, we will seek additional consent from you to do this.

 

Why do we collect, use, hold and share your personal information?

Our clinic will need to collect, use and hold your personal information so that David can provide optimal Naturopathic services to you, and for business purposes.

 

What personal information do we collect?

The information we will collect about you includes your:

 

  • names, date of birth, address, contact details

  • medical information including medical and surgical history, medications, supplements, allergies, adverse events, social history, family history and risk factors

 

How do we collect your personal information?

Our clinic may collect your personal information in several different ways.

 

  1. When you make your first appointment our reception staff will collect your personal and demographic information, usually via phone or by your online registration.

  2. We may also collect your personal information when you send us an email or SMS, telephone us, visit the website to make an online appointment or via social media.

  3. In some circumstances personal information may also be collected from other sources, including from:

  • your guardian or responsible person

  • other involved healthcare providers, such as specialists, hospitals and pathology and diagnostic imaging services.

 

When, why and with whom do we share your personal information?

In rare circumstances we may be required to share your personal data:

 

  • with third parties who work with our practice for business purposes.  These third parties are required to comply with APPs and this policy.

  • with other healthcare providers

  • with Government if an adverse reaction to our treatment occurs

  • when it is required or authorised by law (eg. court subpoenas)

  • when there is a statutory requirement to share personal information (eg. some diseases require mandatory notification)

 

Only people who need to access your information will be able to do so.  Other than in the course of providing naturopathic services or as otherwise described in this policy, our clinic will not share personal information with any third party without your consent.

 

We will not share your personal information with anyone outside Australia (unless under exceptional circumstances that are permitted by law) without your consent.

 

How do we store and protect your personal information?

Client records are securely stored, archived, passed on or disposed of in accordance with applicable privacy and health records legislation.  Storage includes electronic format, in protected information systems, or paper-based records in locked drawers and filing cabinets.  Passwords are changed as required, and all staff and contractors sign confidentiality agreements.

We may ask you to provide us with certain personally identifiable information that can be used to contact or identify you.    By using the Site, you agree to the collection and use of information in accordance with this Policy.  Personal identifiable data may include but is not limited to your name.

Our clinic acknowledges clients may request access to their Naturopathic records.  We require you to put this request in writing and our clinic will respond within 30 days. 

 

Our clinic will take reasonable steps to correct your personal information where the information is not accurate or up to date.  From time to time we will ask you to verify that your personal information held by our clinic is correct and current.

 

How can you lodge a privacy-related complaint, and how will the complaint be handled at our clinic?

We take complaints and concerns regarding privacy seriously.  You should express any privacy concerns you may have in writing.

This Privacy Policy will be reviewed regularly to ensure it is in accordance with any changes that may occur.   We will notify you of any changes by posting the new Privacy Policy on the Site.  You are advised to review this Privacy Policy periodically for any changes.
